The federal government’s Centers for Medicare and Medicaid Services rate more than 15,000 nursing homes on a five-star scale based on inspection results, staffing levels, and various measures to assess quality of care, such as data on injuries from falls, and the use of anti-psychotic drugs. In Feb. 2015, CMS changed their rating criteria, and about one-third of homes lost at least one star on their overall rating.
